Patient-Centered Medical Home (PCMH) is a model of care that puts patients at the forefront of care.  PCMHs build better relationships between patients and their clinical care teams.  PCMHs improve quality and the patient experience and increase staff satisfaction while reducing healthcare costs.

The Georgia Primary Care Association is available to assist our members through the stages of the PCMH recognition process.  We provide technical assistance and training on the standards and guidelines, supportive workflows, reporting needs, and EHR tools that complement program requirements.
