The National Center for Farmworker Health (NCFH) in collaboration with the Association of Diabetes Care & Education Specialists (ADCES) invites you to join us in this 60-minute webinar on diabetes, a chronic condition that requires patients to self-manage their disease, from monitoring their blood sugar to changing their eating patterns to taking medications as prescribed. Building a trusting, collaborative, and person-centered approach to diabetes care with your health center can increase engagement, retention, and self-efficacy to help people with diabetes stay engaged with your diabetes services AND stay healthier, and this webinar will showcase examples of successful and sustainable programs with positive outcomes including short videos and community collaborations.
